Dental Care Grant

Conditions Covered

  • Disease control: dental decay (caries), root canal infection, periodontal disease, infections of the gums and jaws that threaten the longevity of the remaining teeth and/or the systemic health of the individual.
  • Functional compromise: missing posterior teeth, which limit acceptable chewing ability, and missing anterior teeth, which impede normal speech and chewing ability.
  • Esthetic embarrassment: missing or broken anterior teeth, extremely dark teeth due to dental disease, and lack of lip/mouth support, which impedes the ability to engage in social activities and/or employment acquisition. Procedures Covered
  • Dental cleaning, including periodontal scaling and root planing
  • Decay removal, including composite (tooth-colored) and silver fillings, pulpal protection, and crowns and onlays to restore the form and function of the teeth.
  • Endodontic treatment to remove infected pulp tissues and address abscesses.
  • Dental implants to replace teeth.
  • Removable and fixed dentures to replace partial and full edentulism.

Non-covered Procedures

  • Tooth whitening
  • Ceramic veneers
  • Cosmetic fillings to replace acceptable amalgam fillings
  • Gold and ceramic inlays
  • Night guards and sleep appliances
  • Orthodontics
  • Any procedures deemed elective by the grant approval board.

Grant Award Levels

Grants may be made in any amount up to $5,000

Grant a maximum of $5,000 per applicant for life

Grants may only be used for the above procedures, and grant awards will be kept in escrow before distribution directly to the provider.

The Grant Approval Board will determine the level of the grant award

Application Process

1. Eligibility

Ensure that your dental condition falls within the specified conditions covered by the grant, including disease control, functional compromise, and/or esthetic embarrassment.

2. Application Submission

Submit a comprehensive application that includes:

> Personal Information: Full name, contact details, and relevant demographic information

> Dental Condition Details: Clearly outline how your dental condition meets the specified criteria for the grant.

> Procedure Request: Specify the procedures you seek coverage for, ensuring they align with the grant’s covered procedures.

3. Supporting Documents

Include the following supporting documents:

> Dental Records: Provide relevant dental records that substantiate the need for the specified procedures.

> Treatment Plan: Include a treatment plan from a qualified dental professional detailing the recommended procedures.

> Financial Documentation: Submit proof of financial need, such as income statements or tax returns.

4. Application Review

The Grant Approval Board will review applications and assess eligibility based on the specified criteria. Ensure that all information provided is accurate and complete.

5. Grant Award Determination

The Grant Approval Board will determine the level of the grant award based on the severity of the dental condition and the proposed procedures. Grant amounts may range up to $5,000.

6. Grant Distribution

If approved, the grant award will be kept in escrow before distribution directly to the dental service provider. The funds can only be used for the specified covered procedures.

Non-Covered Procedures:

Ensure that your requested procedures are not within the list of non-covered procedures, including tooth whitening, ceramic veneers, cosmetic fillings, gold and ceramic inlays, night guards, sleep appliances, orthodontics, and any elective procedures by the grant approval board.

Grant Award Levels:

Grants may be awarded in any amount up to $5,000. Each applicant is eligible for a maximum grant of $5,000 for their lifetime. The Grant Approval Board will determine the final grant amount by assessing the dental condition and proposed procedures.
